Seniors•At•Home, a division of Jewish Family and Children’s Services, is looking for a well qualified and experienced geriatric care manager/therapist to join our dynamic, team orientated and well established social service organization, to provide direct services to older adults in our Marin County region. We are looking for a highly skilled, experienced geriatric social worker or clinician with advanced training in the area of geriatric mental health who is able to effectively work with a diverse caseload of seniors and their families. You will develop and conduct outreach presentations to community groups, conduct psychosocial and cognitive assessments, and provide direct services, including short-term case management and/or clinical intervention, to an at risk older adult population served by JFCS in the Marin region. This is a .8 FTE benefitted position. Job qualifications are as follows:

1. LCSW or Licensed Psychologist strongly preferred. Will consider MFT with care management experience.
2. Two years of experience providing assessment, care management and mental health counseling to seniors. Knowledge of short term, problem solving treatment approaches preferred.
3. Experience conducting outreach presentations or other training to diverse groups.
4. Ability to form relationships with seniors and their families required.
5. Ability to use the computer as a tracking and planning tool.
6. Ability to work independently and as part of a team.
7. Car and current driver’s license and current automobile insurance.
